Ociperlimab (TIGIT) + tislelizumab (PD-1) with cCRT shows numerically longer PFS vs durvalumab in unresectable stage III NSCLC (AdvanTIG-301)

Ociperlimab is an anti-TIGIT antibody and tislelizumab is an anti–PD-1 antibody; AdvanTIG-301 (Phase III, open-label, randomized, multi-arm; NCT04866017) tested concurrent I/O with concurrent chemoradiotherapy (cCRT) followed by maintenance I/O in treatment-naïve unresectable stage III NSCLC. Before early termination, 63 patients were randomized to ociperlimab+tislelizumab+cCRT → maintenance (arm A; n=22), tislelizumab+cCRT → maintenance (arm B; n=19), or SOC cCRT → durvalumab consolidation (arm C; n=22); PD-L1 ≥1% rates were 68–74% across arms. Median PFS was NR in arm A (95% CI 6.3–NE) vs 15.0 mo in arm B and 10.4 mo in the durvalumab arm C (descriptive only), with ORR 68.2%, 68.4%, and 59.1% (all PRs); grade ≥3 treatment-related TEAEs were similar (~67–68%), while pneumonitis (18.2% vs 9.1%) and ILD (13.6% vs 0%) numerically favored durvalumab—worth flagging given PACIFIC established durvalumab consolidation as the benchmark (mPFS ~16.8–16.9 mo vs placebo) and long-term OS benefit (e.g., mOS 47.5 mo with durvalumab at 4-year update).

Take home

If a TIGIT+PD-1 “all-the-way-through” cCRT strategy is going to displace durvalumab consolidation, it likely needs cleaner lung toxicity and clearly superior time-to-event endpoints in a fully accrued dataset—these early-terminated, small-n descriptive PFS signals (and higher pneumonitis/ILD vs durvalumab) aren’t enough yet to move curative-intent stage III practice.
