Stereotactic radiotherapy (SRT) to all oligo-residual lesions was delivered while continuing maintenance systemic therapy after effective first-line chemo-immunotherapy in a single-arm study from China (driver mutation-negative metastatic NSCLC; n=59; NCT04767009). Median PFS was 29.0 months (90% CI 14.0–NE) and 2-yr OS 88.9% (95% CI 75.9–100); grade≥3 TRAEs occurred in 22%. A prespecified propensity-matched comparison vs contemporary systemic therapy alone favored adding SRT (PFS HR 0.286, p<0.001; OS HR 0.229, p=0.023), which contrasts with the randomized IO-era NRG-LU002 (no PFS/OS benefit for routine local consolidative therapy) yet aligns with pre-IO randomized data (Gomez et al.: PFS 14.2 vs 4.4 mo; OS 41.2 vs 17.0 mo).

Take home

Selection and timing may be everything—if randomized IO-era trials confirm this long PFS with acceptable ≥3 toxicity, consolidative SRT could be a valuable add-on for carefully chosen oligo-residual patients.
