Ociperlimab (anti‑TIGIT) + Tislelizumab (anti‑PD‑1) fails to beat pembrolizumab OS in PD‑L1≥50% 1L NSCLC

Ociperlimab is an anti‑TIGIT antibody combined with the anti‑PD‑1 tislelizumab to deepen checkpoint blockade; AdvanTIG‑302 (Phase 3; NCT04746924) randomized untreated stage III/IV (locally advanced unresectable or metastatic) NSCLC with PD‑L1 ≥50% (5:5:2) to ociperlimab+tislelizumab (A), pembrolizumab (B), or tislelizumab (C) (N=662). At the prespecified interim cut (May 30, 2025) the study was stopped for futility: median OS was 31.9 vs 29.4 months for A vs B (HR 0.97, 95% CI 0.76–1.23), while median PFS/ORR numerically favored A (PFS 14.3 vs 10.5 months; ORR 61.0% vs 48.8%)—but all post‑stop efficacy is descriptive only. Safety looked broadly similar across arms (TRAEs 84.3% vs 79.4% vs 79.3%); importantly, despite higher response/PFS point estimates, the regimen did not improve the endpoint that matters most in this setting, where pembrolizumab monotherapy has long-term benchmark OS around the mid‑20s months in PD‑L1≥50% populations (e.g., KEYNOTE‑024 mOS 26.3 months).

Take home

If TIGIT is going to matter in PD‑L1‑high 1L NSCLC, it probably needs either better patient selection (a “TIGIT‑sensitive” biomarker) or a different combo backbone—because a numerically higher ORR/PFS without an OS delta (HR ~1.0) is unlikely to move practice off pembrolizumab in this population.
