Motixafortide (CXCR4i) + cemiplimab/PD-1 with gem/nab-paclitaxel logs 63% PR and 9.6-mo PFS in 1L metastatic PDAC

In the Phase 1 “signal-seeking” CheMo4METPANC study (NCT04543071), eleven treatment-naïve patients with metastatic pancreatic cancer received motixafortide (CXCR4i), cemiplimab, gemcitabine (1000 mg/m²) + nab-paclitaxel (125 mg/m²). CXCR4 is highly expressed in approximately 33% of PDAC patients. Partial responses were seen in 7 out of 11 patients (63%) and 3 (27%) experienced stable disease (DCR = 91%). . The mPFS was 9.6 months. Grade ≥3 events were mainly anemia (45%) and rash (27%), with universal low-grade skin hyperpigmentation. In comparison, the MPACT regimen of gem/nab alone delivers ~23% ORR and 5.5 mo PFS, and FOLFIRINOX gives a 31% ORR and 6.4 mo PFS—highlighting a sizable efficacy improvement despite the small sample size.

Take home

An encouraging 3-fold jump in response and >4-month PFS gain suggests CXCR4 blockade may finally crack PDAC’s immune shield—Phase 2 randomization will show if the signal holds or fades.
