Tiragolumab (anti‑TIGIT) + Atezolizumab Fails to Beat Durvalumab for PFS in Stage III NSCLC Consolidation

Trial ID

SKYSCRAPER-03 (Phase III, open-label; NCT04513925) tested tiragolumab, a TIGIT-checkpoint monoclonal antibody, plus atezolizumab (PD‑L1 inhibitor) as post–platinum concurrent chemoradiation consolidation versus durvalumab (PD‑L1 inhibitor) in unresectable stage III NSCLC. In the PD‑L1+ primary-analysis set (TC ≥1%; n=209 vs 210), median IRF-assessed PFS was 19.4 vs 16.6 months (HR 0.96, 95% CI 0.75–1.23; p=0.76); in all-comers (n=413 vs 416), median PFS was 14.2 vs 13.8 months (HR 1.00, 95% CI 0.84–1.19), with OS essentially identical (PD‑L1+: HR 0.99; all-comers: HR 0.98). Safety for the combo was reported as consistent with prior experience with no new/unexpected signals; efficacy is broadly in-family with the established PACIFIC durvalumab consolidation benchmark (e.g., 5‑year OS ~43% and 5‑year PFS ~33% in PACIFIC).

Take home

If TIGIT is going to matter in stage III NSCLC, it likely needs a better biomarker (beyond PD‑L1 TC ≥1%) or a different timing/partner—because here the “double-checkpoint” consolidation strategy delivered no clinically meaningful separation from durvalumab on either PFS or OS despite long follow-up.
