Ciltacabtagene autoleucel (cilta-cel) is an autologous anti‑BCMA CAR‑T therapy evaluated in the randomized Phase 3 CARTITUDE‑4 trial (NCT04181827) in lenalidomide‑refractory relapsed/refractory multiple myeloma after 1–3 prior lines, with bridging therapy (PVd or DPd) before a single cilta-cel infusion. In the intent‑to‑treat standard‑risk cytogenetic subgroup, 30‑month PFS was 71.0% (95% CI 58.8–80.2) with cilta-cel (N=69) vs 43.2% (31.3–54.5) with SOC triplets (N=70) at median follow‑up 33.6 months; in the as‑treated standard‑risk group (n=59), 30‑month PFS was 80.5% (67.2–88.8). For a real‑world benchmark in this lenalidomide‑refractory setting, PVd’s reported median PFS is ~9.5 months in lenalidomide‑refractory patients (OPTIMISMM subgroup), underscoring how unusual it is to see ~70–80% of patients still progression‑free at 2.5 years after a one‑time CAR‑T; safety outcomes were not provided in this abstract (so CRS/ICANS/cytopenias can’t be assessed here).

Take home

If these standard‑risk subgroup data hold up with longer follow‑up (and with the safety/bridging attrition transparently reported), cilta-cel looks increasingly like a “treat early for depth/duration” strategy in lenalidomide‑refractory myeloma—something triplets like PVd/DPd rarely deliver beyond ~1 year of median PFS.
