Amivantamab is an EGFR/MET bispecific antibody paired with lazertinib, a third-generation EGFR tyrosine kinase inhibitor, evaluated in Cohort C of the global Phase I/Ib CHRYSALIS-2 study (NCT04077463) in treatment-naïve atypical EGFR–mutated advanced NSCLC excluding exon 20 insertions and co-mutations with classical EGFR (n=49). With median follow-up 31.3 months (cutoff Oct 31, 2025), median OS was 41.0 months (95% CI, 27.7–NE), with 55% alive at 3 years and 46% at 4 years; 20% (10/49) remained on 1L therapy for 2.5–4.4 years, and safety was “consistent with prior reports” (no new signals). This OS compares favorably with the pooled afatinib uncommon-EGFR dataset cited in the abstract (median OS 19.4 months; n=38), though cross-trial differences (mutation mix, subsequent therapy, and early-phase design) limit any direct SOC claim.

Take home

If this ~41-month median OS holds up in larger/controlled datasets, ami-laz could become a serious 1L option for the “uncommon EGFR” group where outcomes with approved afatinib-era strategies have historically been closer to ~2 years—just don’t over-read an early-phase, non-randomized cohort where post-progression therapy was common (71%).
