Durvalumab (PD-L1) + paclitaxel posts ~56% ORR in 1L mTNBC; capivasertib (AKT) or oleclumab (CD73) add little

Durvalumab is an anti–PD-L1 monoclonal antibody combined here with weekly paclitaxel chemotherapy, with exploratory triplets adding capivasertib (a pan-AKT inhibitor) or oleclumab (an anti-CD73 adenosine-pathway antibody), in the BEGONIA Phase Ib/II first-line locally advanced unresectable/metastatic TNBC platform (NCT03742102). Across arms, confirmed ORR was 56.5% with durvalumab + paclitaxel (n=23) vs 54.8% with + capivasertib (n=31) and 51.5% with + oleclumab (n=33)—numerically in the same band as PD-1 + chemo benchmarks in PD-L1–selected 1L mTNBC (e.g., KEYNOTE-355 reports mPFS 9.7 vs 5.6 months for pembrolizumab + chemo vs chemo in CPS≥10), but this abstract does not report PFS/OS, which is what typically differentiates IO regimens. Grade 3/4 AEs were 43.5%, 80.6%, and 24.2% respectively, flagging a substantial tolerability penalty with the AKT triplet without an efficacy offset; responses occurred across biomarker subgroups with a trend toward higher activity in PD-L1–positive disease.

Take home

If PFS/OS don’t separate meaningfully on mature follow-up, these data read more like “durvalumab + taxane behaves like an IO-chemo backbone” than a new triplet standard—capivasertib’s added toxicity especially will need a clearer biomarker-defined win (or a different chemo/AKT schedule) to justify moving forward.
