THYRO-RT (NCT03669432) randomized patients with high-risk resected non-medullary, non-anaplastic thyroid cancer to surgery plus RAI versus surgery plus RAI and adjuvant EBRT (60 Gy/30 fractions). The cohort was very high risk, including pT4a disease in 86.6%, nodal disease in 89.6%, extranodal extension in 71.6%, and R1/R2 resection in 49.3%. At 102 months’ median follow-up, locoregional recurrence was 19.4% with surgery/RAI vs 9.7% with added EBRT (p=0.263), with no 10-year locoregional recurrence-free survival difference and grade 3 acute radiation toxicity in 2 patients.
Take home
This randomized signal argues against reflexively adding postoperative EBRT even in very high-risk differentiated thyroid cancer, though selected R1/R2 or invasive cases may still need individualized discussion.
