Blinatumomab (CD19 BiTE) + ponatinib (BCR::ABL1 TKI) drives 83% CMR and 89% 3-year OS in frontline Ph+ B-ALL

Blinatumomab is a CD19-directed bispecific T-cell engager (BiTE) paired with ponatinib, a potent BCR::ABL1 tyrosine kinase inhibitor active against T315I, in an ongoing single-arm Phase II frontline study in newly diagnosed Ph+ B-ALL (NCT03263572). Among 88 adults (median age 50; 19% >70), ORR was 96% with 95% CR; NGS MRD-negativity was 95% in evaluable patients (43% after cycle 1) and RT-PCR CMR was 83% (67% after cycle 1); at 3-year median follow-up, 3-year EFS/OS were 79%/89% and 79% remained in molecular remission without allogeneic SCT, while relapse clustered in high presenting WBC ≥70×10⁹/L (3-year CIR 31% vs 6.7%; p=0.005). Compared with established chemo-minimizing TKI+blinatumomab approaches such as D-ALBA (dasatinib→blinatumomab; 81% 4-year OS) and SWOG S1318 in older patients (87% 3-year OS), these outcomes look broadly in-family on survival while pushing very high early molecular clearance and a low transplant-in-CR1 rate—watchouts are CNS-only relapses (mitigated after increasing IT therapy) and the need to understand ponatinib vascular risk in long maintenance.

Take home

If the CNS-risk and high-WBC subgroup can be consistently “patched” (more IT therapy and/or added high-dose MTX/cytarabine, maybe even CAR-T for very high-risk), this regimen strengthens the case that many newly diagnosed Ph+ ALL patients can skip upfront allo-SCT without obviously sacrificing 3-year survival versus the best published TKI+blina chemo-sparing benchmarks.
