Rucaparib (PARP inhibitor) maintenance after platinum induction hits 17.7 mo rPFS in BRCA-C (BRCA1/2 or PALB altered) mCRPC

In the single-arm Phase 2 trial NCT02985021, 18 HRR-deficient mCRPC patients received four 21-day cycles of docetaxel 60 mg/m² + carboplatin AUC5, then continuous rucaparib 600 mg BID; overall median rPFS was 8.1 mo, but in the 12 with BRCA1/2/PALB2 (“BRCA-C”) alterations rPFS stretched to 17.7 mo (95% CI 7.5–NR), roughly doubling historic PARPi-alone outcomes; no new safety signals were reported.
Take home
Platinum priming may sensitize BRCA1/2 or PALB altered tumors to PARP blockade. More is needed before this chemo-plus-maintenance strategy can challenge current practice.
