Daratumumab is an anti-CD38 monoclonal antibody added to lenalidomide (IMiD) + dexamethasone (D-Rd) and tested vs Rd in the Phase 3, randomized MAIA trial (NCT02252172) in transplant-ineligible newly diagnosed multiple myeloma (n=737; median follow-up 64.5 months). D-Rd improved median PFS to 61.9 vs 34.4 months (HR 0.55; p<0.0001) and OS (median not reached vs 65.5 months; HR 0.66; p=0.0003), with higher ≥CR (51.1% vs 30.1%), MRD-negativity (32.1% vs 11.1%), and sustained MRD-negativity ≥18 months (16.8% vs 3.3%); these results materially exceed historical Rd benchmarks from FIRST (median PFS ~26 months; OS ~59 months) in a similar transplant-ineligible population. No new safety signals were reported with longer follow-up, and efficacy was maintained across older age strata (including ≥75 and ≥80 years).

Take home

With ~5+ years’ follow-up, MAIA continues to make a strong case that adding CD38 targeting to Rd is not just about deeper responses (MRD) but also durable survival—now the key question is how best to sequence/optimize CD38-based frontline therapy against other current options (e.g., D-VMP) for frailer, very elderly patients.
