Larotrectinib, a highly selective CNS-penetrant TRK inhibitor, was evaluated in two Phase 1/2 trials (NCT02122913, NCT02576431) involving 32 patients with advanced TRK-fusion lung cancer—59% had ≥2 prior therapies and 38% had baseline brain mets. Investigator-assessed ORR was 69% (13% CR); the median PFS was 22 months with a median OS of 41 months. The median duration of response was 34 months. Grade 3/4 treatment-related events occurred in 31%, leading to discontinuation in only one patient.
Take home
These durable, CNS-active responses from larotrectinib in TRK-fusion lung cancer, while found in less than 1% of all lung cancers, still reinforce the importance of routine NTRK fusion testing.
