In the phase 3 CROWN trial (lorlatinib, a ALK inhibitor, n = 149 vs. crizotinib, an ALK and cMET TKI inhibitor, n = 147), 95% of lorlatinib-treated, treatment-naïve ALK-positive NSCLC patients were evaluable for depth of response (DepOR). 3-5% of NSCLC cases are ALK+. Patients achieving >75% tumor shrinkage (34% of the cohort) recorded a 5-year progression-free survival (PFS) rate of 74.8%, compared with 62.3% for the >50–75% group and 36.9% for those with ≤50% shrinkage. Median PFS was not reached in either deep-response group but was 12.7 months for ≤50% shrinkage (HR 0.25 vs reference). Baseline characteristics were well balanced, although brain metastases were slightly more common in the deepest-response group—highlighting lorlatinib’s intracranial potency. Historical crizotinib benchmarks (mPFS = 10 mo) underscore how sustained, deep responses translate into the longest PFS yet reported for any single-agent targeted therapy in metastatic solid tumors.

Achieving >50% tumor shrinkage on first-line lorlatinib predicts multi-year disease control, reinforcing the value of early response as a surrogate for long-term benefit in ALK-positive lung cancer.
