Pooled data from the GRIFFIN and PERSEUS studies in transplant-eligible patients aged ≥65 years indicate that adding daratumumab to VRd induction/consolidation followed by daratumumab–lenalidomide maintenance significantly deepens responses—with higher rates of complete response (82.8% vs. 67.0%) and minimal residual disease negativity (66.4% vs. 41.7%)—and shows a trend toward improved progression-free survival at nearly 50 months follow-up, without new safety concerns.
This analysis supports adopting D-VRd followed by D-R maintenance as a new standard of care in older NDMM patients, reinforcing that age should not deter the use of intensive, daratumumab-based therapy for transplant-eligible individuals.
