In the phase 2 ALEK-B trial, combining alectinib (600 mg BID) with bevacizumab (15 mg/kg) in 41 advanced ALK-rearranged NSCLC patients, including those with brain metastases, resulted in a 97.1% 12-month progression-free survival rate, compared to ~77% for alectinib monotherapy, 100% objective response rate, and favorable 36-month PFS (64.2%) and OS (87.9%) with manageable safety findings and improved quality of life over a 34.5-month median follow-up.

This combination strategy demonstrates robust efficacy and control, including high intracranial activity, where nearly all the patients are alive at a year, and more than half are 3 years out, but also supports further investigation into delaying resistance mechanisms in ALK+ NSCLC.
